Periodic Medical Exams

Periodic medical exams may be completed to support general health monitoring and preventive care. These exams may include:

  • Review of medical history

  • Physical examination

  • Assessment of general health concerns

  • Preventive screening discussions, when appropriate

The frequency of exams is based on individual health needs and physician assessment.

Cognitive Screening (Mini-Mental Exam)

When clinically appropriate, physicians may use brief cognitive screening tools, such as a Mini-Mental examination, to assess aspects of memory, attention, and orientation.

These tools are screening assessments only and do not provide a diagnosis. Further evaluation or referral may be recommended based on findings.

Medical Forms & Documentation

Physicians may assist with completion of non-WCB, non-Driver’s Medical forms when appropriate, including:

  • Employment or program medical forms

  • School or institutional documentation

  • Follow-up assessments related to prior exams

Physical exams are provided by scheduled appointment only and are not available through the walk-in clinic.

Patients are encouraged to contact the clinic in advance to confirm documentation requirements and appointment availability.

Frequently Asked Questions

How often should I have a periodic medical exam?

The frequency depends on your age, health status, and risk factors. Generally, adults should have an annual check-up, but your healthcare provider can give personalized advice.

Who needs a mini-mental exam?

A mini-mental exam is typically recommended for individuals who may be showing signs of memory loss, confusion, difficulty concentrating, or changes in behavior. It is commonly used to screen for conditions such as dementia, Alzheimer’s disease, and other cognitive impairments. Older adults, patients with a family history of cognitive decline, or anyone experiencing difficulties with daily thinking or memory tasks may benefit from this exam. Early detection through a mini-mental exam can help guide timely and appropriate care.

What should I do if I need to reschedule my appointment?
